Qualifying events
To make a change, employees must submit the request during the transfer period or within 31 days of a qualifying event.
Qualifying events include the following:
- A change in family status due to death, birth, adoption, domestic partnership, marriage, divorce, annulment, or legal separation between the employee and spouse
- The attainment of the maximum age for coverage of a dependent child
- A court order requiring an employee to provide health coverage for eligible dependent children
- A change in the employee's residence so that he or she is no longer in the health plan service area
- A change in the employee's title that necessitates a change in health plan
- Loss of a spouse's coverage
- Being out on a leave of absence during a transfer period
