Required documentation

Employees are required to submit required documentation when initially enrolling and adding dependents and when making certain changes to their benefits plan. If the employee fails to submit the required documentation, the requested change will not be approved and implemented.



Dependent addition

When adding dependents (child, spouse, or domestic partner) to a health plan, the employee must submit a copy of one of the following documents to NYCAPS Central:

Child

  • Birth certificate

  • Court papers in case of adoption

  • Legal Guardianship/Custody papers

Spouse

  • Marriage certificate

Domestic partner

Employees must contact the Office of Labor Relations at (212) 306-7605 for information about domestic partners.

Once the required documentation is received and approved by the agency benefits representative or NYCAPS Central, whichever is applicable, the change will process overnight.

Dependent removal

When removing dependents from a health plan as a result of a qualifying event, the employee must submit a copy of one of the following documents to NYCAPS Central:

Child

  • Proof of other coverage

  • Death certificate

  • Marriage certificate (if dropping due to child's marriage)

  • Proof of ineligibility such as reduced school hours resulting in less than full time status

Spouse

  • Divorce decree

  • Death certificate

  • Proof of other coverage

Domestic partner

Employees must contact the Office of Labor Relations at (212) 306-7605 for information about domestic partners.

Once the required documentation is received and approved by the agency benefits representative or NYCAPS Central, whichever is applicable, the change will process overnight.

Important: You do not need to submit required documentation when dropping a dependent during the transfer period.

Name corrections

When requesting a name correction, the employee must submit a copy of one of the following documents:

Dependents

  • Birth certificate

  • Court papers in case of adoption

    Important: Required documentation should be submitted to the agency benefits representative or NYCAPS Central, whichever is applicable.

Name changes require approval and will be reflected one business day after the change was made in NYCAPS through ESS or after the required documentation was received, whichever is later.

Buy-Out Waiver election

When enrolling in or leaving the Buy-Out Waiver, employees must complete the Medical Spending Conversion (MSC) form and return it to their agency benefits representative or NYCAPS Central, whichever is applicable.

Loss of other coverage

When enrolling in City health benefits as a result of loss of other coverage, the employee must submit documentation stating the date of termination of the other coverage.
