Changing your name
Employee Self-Service (ESS) contains each employee's first, middle, and last names, as well as their prefix, and suffix. Employees can change any of this information after they have changed the information with the Social Security Administration and received a new Social Security card. All ESS name changes will be verified with the Social Security Administration.
Note: ESS name change requests are verified with the Social Security Administration. Requests will not be approved if the requested name does not match Social Security Administration records.
To change your name
- From the ESS navigation menu, do the following:
- Click the Employee Self-Service link.
- Click the Personal Information link.
- Click the Name Change link.
- Select from below.
- You are changing your first, middle, and/or last name
Important: You must update this information with the Social Security Administration and receive your updated Social Security card before you proceed.
- Under the New Name section, update your information as appropriate.
Important: Your updates must match your new Social Security card exactly.
- Notify other City agencies as appropriate. This may include any of the following:
- Click the Submit button.
- Click the OK button.
It may take up to two weeks for the change to take effect on ESS.
Note: If your requested name does not match Social Security Administration records, this change request will be denied and you will not see the updated name under the Personal Information pages.
- You are changing only your prefix and/or suffix
- Update your information as appropriate.
- Click the Save button.
